Despite all the daily challenges that we face we still need to create a patient-centered culture. Putting patients first is the underlying principle. It is the little things that are put in place that actually impact the patients experience and care. When you take them all together they begin to change the culture and even though they may not have a huge impact on the macro system, for one person they may make a difference.
We provide inpatient care for older patients who come to the hospital with acute medical conditions. We aim to ensure that as many patients as possible can return home following their hospital stay. Inpatient care is provided in our COE wards. We work as a MDT to provide best possible care to our patients.
The MDT consists of:
• Medical and Nursing Team
• Occupational Therapist
• Rehabilitation support workers
• Social Care Assessors
• Speech & Language Therapist
• Discharge Facilitator
• Diversional Therapist
• Palliative Care Team
• Other Specialist Community
meeting continual communication and progress reports are offered on an open access to patients and families from the Medical and Nursing team
